FLU SHOT CLINIC
The Plainview Health Department is coming to Cotton Center to administer Flu Shots
When: Tuesday, October 23rd
10:00am to 1:00pm
Where: Cotton Center Elementary
Nurse’s office
Medicaid, Medicare, and patients with no insurance, both adult and child will be free, but you MUST bring your card/information with you.
If you want your child to receive a shot, he or she MUST bring or send the COMPLETED forms to school on or before
October 23rd.
CHIP or other insurance patients, both child and adult will be $20*. The Health Department takes both cash and checks.
You must bring/send COMPLETED forms and appropriate money to school on or before the day of the clinic.
Forms will be available at the school on that day for all the adults who wish to receive a shot.
